Each client should fill out the CONSENT TO TREATMENT FORM and Symptom Form before you begin (just as you would in your office). After evaluating their symptom form and making notes of additional questions you needed to ask for a fuller/larger picture of the client, you need to determine what should be done. Create a diet, supplement, herb, and/or homeopathic plan and fill out the Supplement Recommendation Example. Write out what you did and why and send all forms and notes to Sharlene Peterson (edu@genesisschoolofnaturalhealth.org).
